How do you take notes on what you read? Show me your system

I'm trying to remember more of what I read. Right now my 'system' is underlining things in pencil and then never looking at them again.

Do you keep a reading journal? Use an app? Write in margins? Index cards like a 1950s scholar? I'm especially curious how people handle library books where you can't write anything.
